29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access: On or about October 9, 2014, employees performing masonry work accessed and egressed the scaffold by means of the end rails/braces and were not provided proper access exposing employees to a fall hazard up to 20 feet to the ground below.
