29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    a) On or about 10/9/14 in the bailer area, doors on the bailer were damage leaving a 3 1/2 to 4 inch gap exposing employees to caught between/crushed hands and fingers' injuries.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  a) On or about 10/9/14, in the establishment, employer did not have certifications of lockout/tagout (LOTO) periodic inspections of machines such as, but not limited to: Clamp, crimper, rubber press, Bridgeport.     NO ABATEMENT CERTIFICATION REQUIRED
