29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    The employer does not guard the rotating parts of machines to protect the operator and other employees in the machine area.         a.  This violation occurred on or about Oct 14, 2014, on the west wall of the Machine Shop where employees were exposed to struck-by and caught-by hazards when operating an Hurco VM1 CNC Mill Machine, serial number 06031046BHA, without interlocking doors.        b.  This violation occurred on or about Oct 14, 2014, on the south wall of the Machine Shop where employees were exposed to struck-by and caught-by hazards when operating a Shaoxing Metal Lathe, without a chuck guard.          c.  This violation occurred on or about Oct 14, 2014, on the east side of the Machine Shop where employees were exposed to struck-by and caught-by hazards when operating a horizontal belt sander without a guard.    d. This violation occurred on or about Oct 1, 2014, on the south wall of the Machine Shop where employees were exposed to struck-by and caught-by hazards when operating a Kingston KMT-3V Manual Mill without a point of operation guard.    e. This violation occurred on or about Oct 1, 2014, on the south east west wall of the Machine Shop where employees were exposed to struck-by and caught-by hazards when operating a 7 inch Metal Cutting Horizontal band saw, serial number 09092763, without a guard covering the unused portion of the blade.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all machines that have exposed rotating parts are guarded.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    The employer does not ensure that the work rests on offhand grinding machines are adjusted with a maximum opening of one-eighth inch.     a. This violation was observed on or about October 14, 2014 in northeast corner of the machine shop where an employee was exposed to struck-by hazards when operating a Central Machinery 6" Bench Grinder without adjusting the work rest to a maximum opening of one eighth inch between the rest and the wheel.    b. This violation was observed on or about October 14, 2014 in the center of the machine shop where an employee was exposed to struck-by hazards when operating a Toolmaster 6" Bench Grinder without adjusting the work rest to a maximum opening of one eighth inch between the rest and the wheel.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all abrasive wheel machinery are adjusted to a maximum opening of one eighth inch.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    The employer does not ensure that offhand grinding machines a tongue guard is installed and adjusted to one fourth inch or less from the abrasive wheel or that the distance between the wheel and the end of the safety guard peripheral member at the top of the abrasive wheel does not exceed one-fourth inch.      a. This violation was observed on or about October 1, 2014 in northeast corner of the machine shop where an employee was exposed to struck-by hazards when operating a Central Machinery 6" Bench Grinder the without tongue guard adjusted  to a maximum opening of one-fourth inch.    b. This violation was observed on or about October 1, 2014 in the center of the machine shop where an employee was exposed to struck-by hazards when operating a Toolmaster 6" Bench Grinder without the tongue guard adjusted to a maximum opening of one-fourth inch.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all tongue guards on abrasive wheel machinery are maintained to within one quarter inch of the grinding wheel.
