29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:      a) Gerdau Sayreville, NJ: Logistics Staging Yard - Employees exposed to vehicle traffic in the logistics staging area when loading and unloading pallet trucks were not adequately visible at all times.        Observed on/or about 10/14/2014.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):   a) Gerdau Sayreville, NJ: Logistics Staging Yard.  The employers Hazard Assessment for personal protective equipment did not include the visibility of employees in loading and unloading areas. Equipment to include but not limited to visible vests.     Forklift operators are required to move through the logistics staging yard, where employees are exposed to a high rate of vehicle traffic, to load and unload pallet trucks.   Observed on/or about 10/14/2014.

Training on when and how to use personal protective equipment, including but not limited to visible vests to be worn when exposed to vehicle traffic, was not provided.   Observed on/or about 10/14/2014.
