On or about October 06, 2014, employees who were performing framing activities were exposed to tripping and laceration hazards due to the accumulation of debris and material throughout the construction site.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about October 06, 2014, employees were using an extension cord to power a Dewalt saw that had the outer sheathing pulled away exposing the internal wiring. The employees were exposed to electrical hazards.
