29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:      a) At the jobsite, sidewalk - A stairway, ladder, ramp or other safe means of egress was not placed inside two trench excavations that were 12 and 7.5 feet in depth. Two employees were working inside the trenches without means of egress. Observed on or about 06/04/2014.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).       a) At the jobsite, sidewalk - Each employee working inside an underground tunnel connecting two trenches was not protected from cave-in hazards by an adequate protective system. The employees were replacing a lateral sewer line inside a trench measuring approximately twelve (12) feet in depth. Observed on or about 06/04/2014.

29 CFR 1926.652(g)(1)(ii): Shields were not installed in a manner to restrict lateral or other hazardous movement of the shield in the event of the application of sudden lateral loads:      a) At the jobsite, sidewalk - Employees were exposed to cave-in hazards while working inside a trench that was approximately twelve (12) feet in depth. The trench shields used were not installed in a manner to restrict lateral movement. The hydraulic jacks were placed at an incline with one end higher than the other. Observed on or about 06/04/2014.

29 CFR 1926.652(g)(2): Excavation of material to a level no greater than 2 feet (.61m) below the bottom of a shield system was allowed when there were indications while the trench was open of a possible loss of soil from behind or below the bottom of the shield:    a) At the jobsite, sidewalk - The employees were exposed to cave-in hazards when working inside trench excavations approximately four feet below the bottom of the shields. The trench excavations were open to loss of soil due to water contained inside them. Observed on or about 06/04/2014.
