The fan was plugged into 120 volt-ac wall receptacle. The plug on the cord of the fan lacked a ground prong. NO ABATEMENT CERTIFICATION REQUIRED

The fan was plugged into a 120 volt-ac wall receptacle. The outer sheath of the fan cord was frayed at the point where it entered the back of the fan, exposing the conductors. NO ABATEMENT CERTIFICATION REQUIRED
