(a) Allied Roofing LLC at 3500 Swanstone Dr. #1 and #2, Fort Collins, CO 80525: On and before 10/2/14, the exposing employer failed to ensure that employees engaged in roofing activities 6 feet or more above lower levels were protected from falling by use of a fall protection system.  This condition exposed employees performing roofing work to fall hazards in excess of 20 feet.     Abatement Note:  Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Worksheet").
