29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a) Establishment -  On or about 10/14/2014, the employer did not establish or utilize methods for isolating energy sources to disable machines and equipment, such as but not limited to the CNC Machines, band saws and drills, to prevent unexpected release of energy, exposing employees to amputation hazards.

29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinders (right angle head grinders) were not provided with safety guards having a maximum exposure angle of 180 degrees and located between the operator and wheel:    a) Fit-Up Area, On or about 10/14/2014, 14 employees using 32 portable grinders that were not guarded, exposing employees to flying metal chips, sparks, and broken wheel fragments.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) Under the stand-up desk, South East corner; on or about 10/14/2014, there was a Relocatable Power Tap (Power strip) plugged into another. The daisy-chaining of these power strips are not used in accordance with listing or labeling. This potential overloaded circuit can expose employees to shock and fire hazards.
