29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    WORKSITE: The employees engaged in residential roofing activities on a 12 pitch roof were not protected from the hazard of falling in excess of 25 feet by the use of appropriate fall protection systems/devices.

29 CFR 1926.502(d)(15)(i): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two:    WORKSITE: The employee, holding the vertical lifeline as an anchor point of another employee who was engaged in roofing activities was incapable of supporting at least 5,000 lbs. per employee and did not have a safety factor of at least two (2).

WORKSITE: The employee, holding and acting as an anchorage point for the vertical lifeline of another employee who was engaged in roofing activities was not capable of supporting at least 5,000 lbs. per employee and was not done under the supervision o a qualified person.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    WORKSITE: The employees, exposed to fall hazards while engaged in residential roofing activities on a 12 pitch roof were not provided information and training in the hazards associated with falls, or the procedures necessary to eliminate or minimize those hazards.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.  WORKSITE: The employees using a Werner model TF1AO 6-foot type 1A, fiberglass stepladder to access the roof were not provided with information and training in the hazards associated with the type of ladder being used or the procedures necessary to eliminate or minimize those hazards.
