The following machines were operated without machine guarding:   a/  Okuma CNC Machine (Model:  MC-60VAE, S/N:  0252, Supply Voltage  220/440V) was operated without machine guarding to protect employees from rotating parts and flying debris.  b/  Peerless Horizontal Band Saw (Model:  HB - 1014PC) was used by employee(s) with the unused portion of the blade unguarded.  Approximately two inches of the unused portion of the blade was not guarded.  c/  Monarch Model K Lathe (Model:  1610 X 54, S/N:  44334, Machine:  L-18) was operated with an unguarded chuck.  American Pulverizer Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 315338509, citation number 01, item number 012 and was affirmed as a final order on 08 August 2011, with respect to a workplace located at 1319 Macklind Avenue, Saint Louis, Missouri.
