29 CFR 1910.110(d)(8)(i): Valves, regulating, gaging, and other container accessory equipment shall be protected against tampering and physical damage. Such accessories shall also be so protected during the transit of containers intended for installation underground. In the Assembly Department a Propylene Tank (UPC: 058894932) used for welding operations was found to be missing the protective glass covers for the pressure regulator gauges leaving the gauges unprotected from tampering and physical damage.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks: In both the Assembly and Vintage Machinery Departments the operators were not provided guarding to protect them and other employees from hazards associated with in-going nip points, rotating parts, and flying debris. The following machines were operated without machine guarding: a/ Okuma CNC Machine (Model: MC-60VAE, S/N: 0252, Supply Voltage 220/440V) was operated without machine guarding to protect employees from rotating parts and flying debris. b/ Peerless Horizontal Band Saw (Model: HB - 1014PC) was used by employee(s) with the unused portion of the blade unguarded. Approximately two inches of the unused portion of the blade was not guarded. c/ Monarch Model K Lathe (Model: 1610 X 54, S/N: 44334, Machine: L-18) was operated with an unguarded chuck. American Pulverizer Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 315338509, citation number 01, item number 012 and was affirmed as a final order on 08 August 2011, with respect to a workplace located at 1319 Macklind Avenue, Saint Louis, Missouri.
