29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about October 7, 2014, on the south side of the construction site, employees used a skill saw with the power cord outer sheathing pulled away exposing the internal wiring. The employees were exposed to electrical hazards.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used. On or about October 7, 2014, on the south side of the construction site, employees building concrete forms utilized an extension cord with the outer protective sheathing split/cut to power a skill saw exposing employees to electrical hazards.
