29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide:     This violation occurred on or about October 16, 2014 at a jobsite located at 303 Oxford Street in Coppell, Texas 75019; where at least two (2) employees were working on the second level of a tubular welded frame scaffold whose platform was not fully planked and had gaps larger than 1 inch between adjacent planks. The employees were not protected from falling approximately 12 feet to the ground below.

29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired:    This violation occurred on or about October 16, 2014 at a jobsite located at 303 Oxford Street in Coppell, Texas 75019; where at least two (2) employees were working on the second level of a tubular welded frame scaffold whose platform contained a defective board that had not been immediately repaired, replaced, braced, or removed from service until repaired.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Scaffold end frames were used as a means of access:    This violation occurred on or about October 16, 2014 at a jobsite located at 303 Oxford Drive in Coppell, Texas 75019; where at least two (2) employees were working on the second level of a tubular welded frame scaffold and were accessing the scaffold by climbing the end frames. The employees were exposed to falling approximately 12 feet to the ground below.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity):    This violation occurred on or about October 16, 2014 at a jobsite located at 303 Oxford Drive in Coppell, Texas 75019; where at least two (2) employees were working on the second level of a tubular welded frame scaffold and did not have a guardrail system or personal fall arrest system in place. Employees were exposed to falling from the open ends of the scaffold to approximately 12 feet to the ground below.
