29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    a.) Facility - On or about 10/16/14, maintenance employees to repair/maintain rooftop equipment such as but not limited to the rooftop exhaust fans and HVAC units, exposing employees to falls of up to 15+ feet to the ground.  The employer has not guarded open-sided floors that are more than four feet above ground level with standard railings (or equivalent fall protection).    ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a.) Facility - On or about 10/16/14, maintenance employees perform repair and replacement of multi-energy facility equipment such as, but not limited to: Lochinvar Hot Water heater (1million BTU gas, 110 V electric, HW); Raypac Hot Water Heaters (1 million BTU gas, 120 V electric, HW); Speed Queen Dryers (gas and electric)  The employer had step by step procedures developed for the equipment, but had not conducted a periodic inspection of the energy control procedures at least annually to ensure that the procedure and the requirement of the standard were being followed.  ABATEMENT CERTIFICATION REQUIRED
