29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a. On or about October 16, 2014 - at the above addressed jobsite, employees performing masonry operations from a tubular welded frame scaffold outrigger plank were exposed to injuries from falls of more than ten (10) feet to the lower deck, due to lack of side rails.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about October 16, 2014 - at the above addressed jobsite, employees working on the second floor deck in the material handling area, approximately twenty three (23) feet above the ground, were not protected from falls, thereby exposing the employees to fall hazards.
