29 CFR 1910.119(h)(3)(ii): The contract employer did not assure that each contract employee was instructed in the known fire, explosion, or toxic release hazards related to his/her job and the process, and the applicable provisions of the emergency action plan. Employees performed work activities during turn around operations, at the Formosa Chlor-Alkali Unit, were not provided adequate training on the possible hazards they could encounter prior to being exposed to hazardous chemicals in the following instances: a. October 7, 2014, at the Chlor-Alkali Unit, employees conducting crane operations were exposed to a chlorine vapor cloud as maintenance was performed on a seal leg sight glass system. b. October 12, 2014, at the Chlor-Alkali Unit, employees performing crane operations were exposed to a chlorine vapor cloud when bolts securing the pressure control valve failed, separating the pressure control valve between the bonnet and the valve body, allowing hydrochloric acid to discharge into the atmosphere and exposing employees to a hydrochloric vapor cloud.
