29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  On October 20, 2014 at this job site, employees worked from an Upright, TL38 Serial#1372-8000 aerial lift, without attaching a lanyard from a harness or body belt to the boom or basket. The employees were exposed to being ejected or falling from the basket at heights up to 20 feet.
