During repair of the  Pulva-Sizer Size B mill, an employee used a Fluke 77 Series II Multimeter to test 240 Volt three phase circuit conductors inside of the A-B  Grinder Side B # 4067 switchbox to determine if the Techtool Premium 20 Horsepower motor was correctly powered.        Violation occurred on or around  8-15-14.    b) The employer failed to perform a hazard assessment for a mechanic performing live voltage testing on equipment using only safety glasses.   When the mechanic tried to repair the Model Number A-2 and Serial Number 2056 Pulva-Sizer Size A mill, the worker used an Amprobe Ultra Amperage meter inside of the A-B switchbox to determine if the  Pulva-Sizer 5 horsepower motor was correctly powered.     Violation occurred on or around 9-15-14.

Abatement Note:   Refer to and comply with 2015 NFPA 70E requirements stated in Table 130.7(C)(15)(A)(b) Arc-Flash Hazard PPE Categories for Alternating Current (AC) Systems:  For approach to an Arc Flash boundry of 19 inches or less for work on 240V or below is defined as Category 1, requiring  Arc-Rated Clothing, Minimum Arc Rating of 4 cal/cm2,  Arc-rated long-sleeve shirt and pants or arc-rated  coverall Arc-rated face shield, or arc flash suit hood,  Arc-rated jacket, parka, rainwear, or hard hat liner, Protective Equipment Hard hat, Safety glasses or safety goggles, Hearing protection , Heavy duty leather gloves, and Leather footwear.  If the employee approaches said circuits within the restricted boundry, appropriately insulated tools are also required.  Please see additional notes section in NFPA for this list of equipment.
