On or about 10/16/14, at the above addressed jobsite, employees were exposed to caught-in-between hazards due to a belt sander (Jet Woodworking Sander) not being securely anchored to the ground.  b.  On or about 10/16/14, at the above addressed jobsite, employees were exposed to crushing hazards due to a drill press (Central Machinery / Model #39955) not securely anchored to the ground.   c.  On or about 10/16/14, at the above addressed jobsite, employees were exposed to amputation hazards due to a band saw (Strong Arm / Model #14) located near employee's break room that was not securely anchored to the ground.   d.  On or about 10/16/14, at the above addressed jobsite, employees were exposed to amputation hazards due to a band saw (Strong Arm / Model #14) located near the rear exit that was not securely anchored to the ground. On or about 10/16/14, at the above addressed jobsite, employees were exposed to shock and electrocution hazards while working with a welding machine (Intervec / Model #V310-T) that had a damaged outer jacket insulation cable with exposed wires.    b.  On or about 10/16/14, at the above addressed jobsite, employees were exposed to shock and electrocution hazards while working with a band saw (Strongarm / Model #39955) that had a damaged outer jacket insulation cable with exposed wires.    PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
