29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about October 15, 2014 and times prior thereto, employees of Enable Midstream Partners, LLP performed pigging operations for the Fort Hill natural gas distribution pipelines located at or near 409 Ashley 88 Road in Hamburg, Arkansas. Employees climbed the blow-down pipe stack of mainline 3 for adjustment of the valve setting for PLC-1000 (Pipe-Line Cleaner) being gravity fed through the 4 inch blow-down opening, exposing employees to fall hazards of greater than 4 feet.

The employer had not ensured employees working with liquid chemicals such as but not limited to PLC-1000 were protected from splash hazards with the appropriate eye protection.

The employees were exposed to chemicals to include but not limited to PLC-1000. The employer had not provided employees with effective information and training on the hazardous chemicals in their work area at the time of their initial assignment.
