29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:  a) For the Brew Hub establishment at 3900 S Frontage Rd., Lakeland, FL, as observed on or about 10/16/2014, confined spaces were present having hazards such as, but not limited to, potentially hazardous atmospheres and mechanically moving parts that could injure and/or entrap employees. No signs were posted informing that the confined spaces were permit required and no other means had been taken to notify employees of the hazards of confined spaces that were present.
