29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  (a)  Beneath the scaffold - On or about October 10, 2014 employees were exposed head injuries while other employees were working on a scaffold above them handling bricks.  None of the employees on-site were wearing hardhats.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:   (a)  Left side of the house - On or about October 10, 2014 employees were exposed to a fall hazard due to a scaffold not being fully decked or planked. There were spaces in between boards resulting in a 16.5 inch gap where employees could fall 13 feet 11 inches to the ground.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  (a)  Left side of the house - On or about October 10, 2014 employees were exposed to a fall hazard while working on a scaffold that had no ladder installed. Employees climbed the frame 13 feet 11 inches above the ground to access the scaffold.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  (a)  Left side of the house - On or about October 10, 2014 employees were exposed to several fall hazards due to the fact the scaffold had a damaged platform and boards. This scaffold was not inspected for visible damage prior to use.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.  (a)  Left side of the house - On or about October 10, 2014 employees were exposed to a fall hazard while working on a scaffold 13 feet 11 inches above the ground with no fall protection.
