29 CFR 1926.502(b)(11): When guardrail systems are used at holes, they were not erected on all unprotected sides or edges of the hole. DeCA Commissary Warehouse: On or about October 16, 2014, DeCA commissary warehouse employee(s) were exposed to slip, trip, and fall hazards when a section of the construction barricade was missing at the area where the concrete flooring was demolished creating a large floor hole and there was no guardrail erected on all unprotected sides.
