29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    The company had not developed nor implemented a energy control program consisting of energy control procedures, authorized and affected training and periodic inspections for two sheeter machines with electrical and pneumatic energy sources, two bandsaws, paper shear (cutter) and radial arm saw machine with only an electrical energy source.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    In the Production Area, on Sheeter Machine A, an operator was exposed to the pinch point hazard on three, 1/2 inch pulley belts to a 5 hp motor that was replaced and the guard was removed but not reinstalled.

Gears shall be guarded in accordance with one of the following methods:    In the Production Area, on Sheeter Machine B, an operator was exposed to a pinch point hazard from the two tooth metal gears located on the side of the machine where the operator stands to feed the paper in the tension roller during set up and during machine operation.

29 CFR 1910.305(g)(1)(iv)(A): As a substitute for the fixed wiring of a structure;  In the Production Area, on Sheeter Machine A, a 110 Volt AC  extension cord set was being used and a substitute for fixed wiring for power to a paper counter machine which did not move because it was a part of the operation of the Sheeter machine.
