29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Smith General Contracting, Inc. had employees exposed to fall hazards while accessing and exiting the roof via extension ladders that did not extend at least 3 ft. above the upper landing surface of the roof, on a residential construction site in Springfield, Illinois. These instances included: a. Extension ladder approximately 6 in. above the upper landing surface of the roof and b. Extension ladder approximately 1.5 ft. above the upper landing surface of the roof. Smith General Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number: 414823, citation number 1, item number 3a and was affirmed as a final order on June 12, 2012, with respect to a workplace located at 1618 E.
