29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a. On or about October 16, 2014, on the west side of the residential house, employees were installing Tyvek siding plastic paper on the exterior wall while working on a bracket scaffolding and were not protected from a 24 feet fall hazard by use of a fall arrest system, exposing them to a fall hazard. b. On or about October 16, 2014, on the northwest corner third floor, an employee was working from the upper balcony of the residential house and was not protected from a 20 feet fall hazard by use of a fall arrest system, exposing him to a fall hazard. c. On or about October 16, 2014, on the north side third floor, employees were working in two separate unguarded area at heights of 11 feet 7-inches and 10 feet 2-inches and were not protected by use of a fall arrest system, exposing them to a fall hazard.
