29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about October 15, 2014, and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances:     1) At the press brake area, a press brake being operated via a foot pedal did not have guards provided in the front and the side to protect the operator and other employees in the area from the point-of-operation.    2) In the assembly building, an iron worker being operated which did not have guards provided at the shear and bender locations.    Employees were exposed to amputation hazards.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:    On or about October 15, 2014, and at times prior thereto, in the maintenance shop and throughout facility the employer allowed employees to operate abrasive wheel grinding machines which either did not have tongue guards provided or the opening between the tongue guard and the wheel was greater one-fourth inch.  Employees were exposed to flying fragments in the event of an exploding wheel.

Employees were exposed to flying fragments in the event of exploding abrasive wheel.
