29 CFR 1904.29(b)(1):  A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:  (a)  On the OSHA 300 Log for calendar year 2014, for the press assistant with pulled back:         1.  The employer did not provide the required information on the object or substance that directly injured or made the              person ill.  (b)  On the OSHA 300 Log for calendar year 2014, for the slitting person with a laceration:         1.  The employer did not provide the required information on the object or substance that directly injured or made the              person ill; and         2.  The employer did not provide the days for the job transfer or restriction.  (c)  On the OSHA 300 Log for calendar year 2014, for the lam person with a cut arm:         1.  The employer did not provide the required information on the object or substance that directly injured or made the                person ill; and         2.  The employer did not provide the days away from work.  (d)  On the OSHA 300 log for calendar year 2014, for the press person with a strained left arm:         1.  The employer did not provide the required information on the object or substance that directly injured or made the               person ill.  (e)  On the OSHA 300 log for calendar year 2014, for the press person with a pulled groin:         1.  The employer did not provide the required information on the object or substance that directly injured or made the              person ill.  (f)  On the OSHA 300 log for calendar year 2014, for the MH person with a pulled arm:         1.  The employer did not provide the required information on the object or substance that directly injured or made the              person ill;         2.  The employer did not classify the case whether it was a death, days away from work, job transfer or restriction, or              other recordable case; and          3.  The employer did not provide the number of days the injured or ill worker was away from work or on the job transfer             or restriction.  (g)  On the OSHA 300 log for calendar year 2013, for the slitting person with a cut finger:         1.  The employer did not provide the required information on the object or substance that directly injured or made the              person ill;         2.  The employer did not classify the case whether it was a death, days away from work, job transfer or restriction, or              other recordable case; and          3.  The employer did not provide the number of days the injured or ill worker was away from work or on the job transfer             or restriction.
