29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) 2201 Ridgewood Rd, Wyomissing PA - Employees were not provided with fall protection while walking up and down the steep portion of a flat roof and were exposed to a 9 foot - 3 inch fall hazard to the ground below, on or about October 17, 2014.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:  a) 2201 Ridgewood Rd, Wyomissing PA - Employees were observed conducting roofing operations on a flat roof and were not protected from a 9 foot - 3 inch fall hazard to the ground below, on or about October 17, 2014.  No abatement certification or documentation required.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee exposed to fall hazards to enable them to recognize the hazards of falling and the procedures to follow in order to minimize them:  a) 2201 Ridgewood Rd, Wyomissing PA - Employees were not trained in appropriate fall protection methods to avoid hazards associated with working on roofs, on or about October 17, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
