29 CFR 1926.502(i)(4): All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard:  (a) Throughout the Building - The concrete floor had holes 12 inches deep where the structural steel columns of the building would get set.  These holes had covers; however, the covers did not have any color coding or markings, such as "HOLE" or "COVER," exposing employees to the hazard of stumbling into the hole if the cover got removed and used for another purpose, on or about October 17, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
