29 CFR 1910.132(d)(1)(i): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitate the use of personal protective equipment (PPE) and when the hazards were likely to be present, the employer did not select, and have each affected employee use, the types of PPE that would protect the employees from the hazards identified:                                a) On or about October 17, 2014, at the above located workplace, employees required to work/act as mobile paintball gun targets (charging zombies) were exposed to struck-by hazards including, but not limited to bruises and lacerations. The employer did not adequately assess the workplace for all hazards and did not select and have each affected employee use the type of personal protective equipment that would protect all parts of their bodies including, but not limited to, the hands, necks and sides of the bodies subject to being struck-by paintballs.
