29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into holes by covers: a) On the second floor east end of the building, four (two 8" , one 3" and one 5") holes were in the floor without covers to prevent trips or falls. This violation was observed on or about October 17, 2014. b) On the second floor west end of the building, fourteen (four 8" and ten 5") holes were in the floor without covers to prevent trips or falls. This violation was observed on or about October 17, 2014.
