The employer failed to provide certification of abatement for a Citation and Notification of Penalty issued 11/17/14 that became a final order of the Review Commission on 6/24/16.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about October 17, 2014, at the Non Ferrous Area, employees were exposed to injury while operating a Bobcat S360 Skid Steer Loader that did no have an active back-up alarms.
