Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) Construction Site: On or about October 20, 2014, and at times prior, employees were exposed to a fall of approximately seventeen feet to the ground below while installing composite asphalt shingles on a 4:12 pitch steep slope roof.
