29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening.    On or about 20 October 2014, and at times prior thereto, employees performed maintenance duties from two elevated platforms with unguarded ladderway floor openings, exposing the employees to a fall hazard of approximately 104 inches or 8 feet, 8 inches to the concrete floor below.  1) The work platform for the filter bed has a ladder for access in which the opening is not guarded and is approximately 8 feet, 8 inches from the concrete floor below.  2) The work platform for the cooling tower has a ladder for access in which the opening is not guarded and is approximately 8 feet, 8 inches from the concrete floor below.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinder is designed to accept a cap, shall always be in place, hand-tight, except when cylinders are in use or connected for use.    On or about 20 October 2014, and at times prior thereto, two full cylinders in storage within the plant did not have the valve protection caps on them, exposing the employees working within the area to a struck by hazard due to the valve and stem suffering damage and a catastrophic release of the compressed gas.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.    On or about 20 October 2014, and at times prior thereto, fire extinguishers mounted on the wall near areas A10 and A12 of the production floor were not identified so that they would be readily accessible, exposing the employees working in the area to various fire hazards.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910.     On or about 20 October 2014, and at times prior thereto, a stack of wood dunnage was stored under or within 36 inches to the front of an electrical breaker panel, control panel, and disconnect switch thus blocking access and exposing employees to electrical hazards of approximately 480 volts.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.    On or about 20 October 2014, and at times prior thereto, employees utilized hand-held power tools at a work bench within the Maintenance Shop in which the electrical outlet had a missing knockout, exposing employees to electrical hazards of approximately 120 volts.
