29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a) On or about October 15, 2014, and at times prior, the employer did not ensure that where employees were exposed to injurious corrosive materials, suitable facilities for flushing of the eyes and body were provided for immediate emergency use. The emergency eye wash station, in the maintenance shop, was not operational and no other suitable facilities were available in case of emergency.    1- Employees in the parts department were exposed to injurious corrosive materials, such as Sulfuric acid, from car batteries.        2- Employees in the detailing areas were exposed to injurious corrosive materials, such as Sodium Metasilicate, from a heavy duty degreasing detergent named "Clean Parts all metal safe spray cabinet detergent". 29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    (a) On or about October 15, 2014, and at times prior, the employer did not ensure that modifications and additions which affect capacity and safe operation of forklifts were performed with manufacturer's prior written approval.  Employees were exposed to struck by hazards, in that a Hyster 60 XL forklift was fitted with an in-house made fork attachment to push disabled vehicles into the shop. Fork attachment was not approved by the manufacturer prior to use. A compressed air nozzle in the automobile service shop was observed with a tip that allowed air pressure in excess of 140 p.s.i., exposing employees to air injection hazards. An electrical cable connecting power to the water heater in the wash bay showed signs of strain, the outer insulation at the fitting was damaged, exposing the inner wire insulation. Unlabeled plastic bottles containing heavy duty degreasers, and cleaners were observed in the detailing bay.
