29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) 1130 Commonwealth Building in Tupelo, MS - On or about October 3, 2014, two employees were 18 to 20 feet above the ground and were not wearing a body harness and were not attached to the basket while applying stucco to the building.
