29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  In the warehouse area at 300 Griffith Road Chicopee, MA:  On or about November 17, 2014, the employer did not ensure sufficient access and working space about the electrical equipment, located between the loading dock/warehouse area and an injection molding room, due to a 21 inch high metal railing installed around the sides and front of the electrical equipment without an accessway, plastic tubing and a fiberboard container lid, which did not allow ready and safe operation and maintenance of the equipment.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  In the shipping area at 300 Griffith Road Chicopee, MA: On or about October 21, 2014, the employer did not ensure that an employee using an expanding foam machine was provided effective information and training on the hazardous chemicals in the work area, specifically polymeric Diphenylmethane Diisocyanate (MDI) and MDI monomer.
