29 CFR 1910.132(a): Employer did not provide personal protective equipment to be used when necessary whenever hazards capable of causing injury or impairment were encountered. Employees were exposed to fall hazards of up to 20 feet while performing tree trimming operations and the employer did not ensure fall protection was being utilized. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wore a protective helmet when working in areas where there is a potential for injury to the head from falling objects: Employees were exposed to struck-by hazards due to falling objects, including a chain saw, tree-felling, and trimmed waste. The employer did not ensure employees had been provided with head protective equipment while walking and standing within the radius of the drop zone. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
