29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy: On or about October 6, 2014: a) Employees were exposed to electrical shock or burns while utilizing a flexible cord to power a sump pump, that was plugged into an outlet that did not have a faceplate installed, to protect live circuits.

29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures. On or about October 6, 2014: a) An employee was injured when the concrete sidewalk he was operating on broke away, causing the equipment in use to fall into the open excavation. The employer failed to properly provide a support system to protect employees from such a collapse from happening.
