29 CFR 1926.652(b)(2): Option (2) - Determination of slopes and configurations using Appendices A and B. Maximum allowable slopes, and allowable configurations for sloping and benching systems, shall be determined in accordance with the conditions and requirements set forth in appendices A and B to this subpart. (a) Aslan Construction @ 3400 S Tamarac Drive, Denver CO 80231: On or before October 16, 2014, the employer did not design the slope of the trench in accordance with requirements set forth for Type B soils. The maximum allowable angle for the slope is a 45 degree angle, ie 1:1 slope. The measured angle of the slope was approximately 67 degrees. This condition exposed employees to cave-in and struck-by hazards associated with trenching operations.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed. (a) Aslan Construction @ 3400 S Tamarac Drive, Denver CO 80231: On or before October 16, 2014, the employer provided an A-frame step ladder which was closed and leaned up against the East face of the trench to be used for egress from the trench.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards. (a) Aslan Construction @ 3400 S Tamarac Drive, Denver CO 80231: On or before October 16, 2014, the employer did not ensure that all employees received training for ladder safety requirements, particularly the proper construction, use, and placement of ladders for safe egress from a trench.
