(a) Canine Fun and Fitness Center @ 5390 East Evans Avenue, Denver CO 80222: On or before October 21, 2014 the employer did not conduct a hazard assessment to determine required PPE for all chemicals used in the facility. PPE was made available but employees did not know when it was required according to Safety Data Sheets. This condition potentially exposed employees to chemical hazards.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes: (a) Canine Fun and Fitness Center @ 5390 East Evans Avenue, Denver CO 80222: On or before October 21, 2014 the employer did not require employees to use the hand protection available at the worksite to prevent skin absorption of harmful substances such as but not limited to; Regal Oxy-Klear and Regal Algaecide 60. This condition exposed employees to dermal chemical exposures.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following: (a) Canine Fun and Fitness Center @ 5390 East Evans Avenue, Denver CO 80222: On or before 21 October,2014 the employer did not provide a written hazard communication program which explains how the chemical hazards, PPE requirements, and training will be communicated to employees. This condition exposed employees to chemical hazards associated with various cleaning and disinfecting chemicals used in the workplace.

29 CFR 1910.1200(f)(5): Except as provided in paragraphs (f)(6) and (f)(7) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with the following information: (i): Identity of the hazardous chemical(s) contained therein; and, (ii): Appropriate hazard warnings which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. (a) Canine Fun and Fitness Center @ 5390 East Evans Avenue, Denver CO 80222: On or before 21 October,2014 the employer did not have all secondary containers of chemicals labeled. WYSIWASH disinfectant spray bottles were labeled only as "Disinfectant" and did not provide appropriate hazard warnings. WYSIWASH solution is made by adding a concentrated tablet to water; the solution contains <0.01% Hypochlorous Acid with a pH 5-7.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets. (a) Canine Fun and Fitness Center @ 5390 East Evans Avenue, Denver CO 80222: On or before October 21, 2014 the employer did not provide employees with Hazard Communication training for all the chemicals used at this facility. Of particular concern is the "Regal OxyKlear" powder, which contains potassium sulfates. Potassium sulfates are strong oxidizers and corrosive when dissolved in water. SDS states that water should never be added to OxyKlear powder and that OxyKlear powder must be added to large amounts of water. Employees who are not informed of this hazard, may be at risk of causing a violent reaction, explosion, or fire hazard.

29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. (a) Canine Fun and Fitness Center @ 5390 East Evans Avenue, Denver CO 80222: On or before 21 October,2014 the aisles in the back storage room were partially obstructed by materials and equipment that did not fit on the shelves. These items were removed and placed on top of shelves during the inspection.
