29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  Front of building, On or about October 17, 2014, employees were exposed to a fall hazard of more than 2 feet while accessing the second level (approximately 12 feet high work location) of the scaffolding which did not have a ladder or other means installed to aid the employees in accessing the work location.

Front of building, On or about October 17, 2014, employees were exposed to a fall hazard of more than 10 feet to the dirt ground below while working from an scaffold applying stucco to the exterior of the building and the scaffold did not have standard guardrails installed.
