29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations: (a) Zorro Pad, 350 Kinder Road, Scenery Hill, PA: On or about October 19, 2014, the employer did not have its employees working from the Centrifugal Services Inc. (CSI) Model WSM-03 Vertical Cuttings Dryer work platform to help assemble the incoming auger into place at the top of the dryer. Three employees were working at assembling the dryer and at least one of the three employees was standing on the 3 inch by 3 inch by 1/2 inch wide clips (ears) that were welded to the supporting posts of the machine. An employee lost his balance while standing on two of these clips (ears) and went to step back off of the clips (ears). In this process, the employee caught the top of the steel drip pan edge (with his one foot), which is less than a 1/16 inch thick and in the process lost his balance, fell and broke his ankle.
