29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a.  On or about October 21st, 2014, at the above addressed job site, employees were not protected from falling from a 12 feet canopy on a commercial building.

On or about October 21st, 2014, at the above job site, an employee was not knowledgeable in recognizing fall hazards while working near the edge of a canopy of a commercial building.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a. On or about October 21st, 2014, at the above addressed job site, an employee was exposed to a fall hazard when using an extension ladder which did not extend at least 3 feet above the upper landing surface while gaining access to the roof of a  canopy on a commercial building.
