29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about October 21, 2014 at a jobsite located at 10810 Executive Center Drive, Danville Building, Little Rock, Arkansas, an employee was working in an elevated bucket without the use of a personal fall arrest system, exposing him to the hazard of falling or being thrown from the bucket.
