29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:      a) Jobsite, 1901 Lombard St., Philadelphia - The work area around the scaffolds at the front of the building, including building entrances was not kept free of bricks and debris, exposing employees to trip hazards.  Observed on or about 10/17/14.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a) Jobsite, 1901 Lombard St., Philadelphia - A ladder or other safe means of access was not provided to employees performing masonry operations from a fabricated frame scaffold.  An employee used the side frames and cross-braces as a means of access/egress, thus being exposed to fall hazards up to approximately twenty six (26) feet above ground level. Observed on or about 10/17/14.    SETAG CONSTRUCTION CO., INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.451(e)(6) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 946153, CITATION 1 ITEM 1, AND WAS AFFIRMED AS A FINAL ORDER ON 12/04/13 WITH RESPECT TO A WORKPLACE LOCATED AT 160 DEKALB PIKE, KING OF PRUSSIA, PA 19406.
