(a) Nole Commercial Lighting dba Sign Me Up on or about October 22, 2014, at the address of 493 W. Market St. Tiffin, OH: The employer failed to ensure employees operating from an articulating truck mounted man lift worked from the floor of the basket. An employee was exposed to an approximate 12 ft. fall while standing on the midrail of the basket during the installation of a store front sign.

Market St. Tiffin, OH: The employer failed to ensure employees working from an articulating truck mounted man lift were adequately protected from falls. The employees repositioning a sign from the lift in this condition were exposed to an approximately 12 ft. fall to the pavement below.
