On or about October 20, 2014, employees performing installation activities accessed the roof area with extension ladders that the side rails did not extend at least 3 feet above the upper landing surface exposing them to a fall hazard of approximately 15 feet to the concrete below. a) Werner extension ladder, on the south side of restaurant, facing north, positioned below the guttering. b) Werner extension ladder, on the north side of restaurant, facing south, next to side walk, positioned below the guttering.
