29 CFR 1926.21(b)(6)(i): Employees required to enter into confined or enclosed spaces were not instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required: (a)Rio, LLC at 120th Ave. and Holly St., Thornton, CO 80241: On and before October 22nd, 2014, the creating, exposing, and correcting employer did not train employees who were required to enter into manholes to connect sewer pipe and to grind the grout regarding the nature of the atmospheric and electrical hazards involved, and the means of rescue. The condition exposed employees to atmospheric and electrical hazards.
